Parental/Guardian Release Form: This brief document—which must be signed by all Make a Connection Thru Art participants and returned to Make a Connection staff—affirms that each student’s parents/guardians understand that their child is participating in the Make a Connection Thru Art program. The Release Form also gives limited rights for use of the child’s image and/or artwork in conjunction with the Make a Connection program.
